Weak mount screw/threads

I owned 4 pairs total. Got them when Brownell's was selling them for $99 dollars. Really like the design of the rear site. Doesn't interfere at all with charging handle. But recently I went to remove one and noticed half of the slotted flat-head mount screw had sheared off. I managed to get it off, but the helical thread insert came out with it. I never use excessive torque. Very experienced motorcycle and aircraft mechanic. I don't over-torque fasteners!
