I have used Brownells forms for years and have found them to be precisely what is needed, from vitals of the owner as well as the firearm. FINALLY the carbon has been eliminated by the RCA paper. Thanks, less mess and waste.

3-part form
These forms are well laid out. I use these in my shop and makes it easy to get all the information on the form. I always give a copy to the customer as a receipt when they leave their firearm. Plenty of room to put what the gun is in for and what you did to the firearm.
The best repair form around
These are the repair forms we have used in the shop since we opened. They are well laid out, and provide plenty of room for the shop guys to make notes. The real star is at the bottom of the tag. There's a space for any extra accessories that was left with the firearm. Magazines, scope, case, ammo, misc. These are a life saver when the customer comes back and says he "thinks" he left his $128 Alessi Holster here too, and we can show him on the ticket that it was NOT left, or we can go find it for him if it was! The 3 copies also help, as the customer can take one, accounting gets one, and the shop has a HEAVY stock copy on the back. The heavy final page resists tearing, and can take a beating!

Very well designed form. Perfect for gun shop work.
Gets the job done
I personal like the larger form. Works well with a metal contractors clipboard. Has nice area at the top to but you business header. Like that it doesn't have the pre-printed numbers so you can put your own. Would prefer a 3 part from so you could give the customer a copy when they drop off and a completed copy for receipt at pick-up. Also for such a large form a larger comments section would be preferable. Bottom line is it works.
